General Instructions: This form is to be completed by a Michigan resident when purchasing from a Federal Firearms Licensed (FFL) dealer or when a purchaser has a valid Michigan Concealed Pistol License (CPL). A purchaser who is an FFL dealer shall also complete this form.

Distribution: Provide one copy to the seller and one copy to the purchaser. Within 10 days of purchase, the MSP copy must be submitted by the purchaser to the police or sheriff department having jurisdiction where the purchaser resides. The law enforcement agency is required to forward that copy to the Michigan State Police.

Instructions: If more than one pistol is purchased or acquired, do not use the first three lines of the Pistol Sales Record. Instead, indicate the number of pistols purchased or acquired in the first box and list all pistols on an invoice or bill of sale. The invoice or bill of sale must contain all of the information that is shown in the first three lines of the Pistol Sales Record. Attach a legible copy of the invoice or bill of sale to each copy of the Pistol Sales Record.
